This surgical intervention aims to safely drain and clear the cyst structure and its contents formed by the parasite. The main goal of treatment is to stop the damage the cyst causes to the liver and surrounding tissues and to prevent serious complications such as rupture or inflammation. Surgery is still regarded as the most definitive and reliable “gold standard” method in the treatment of this disease, especially in advanced or complicated cases.

Contact Us.

You can request an online appointment by filling out the form. Thanks to advances in liver surgery over the last 15–20 years, selected hydatid cyst cases can now be successfully treated with a “closed,” i.e., laparoscopic method.

In this method, instead of making a large incision in the abdominal wall, a camera and special surgical instruments are inserted through 3–4 small (0.5–1 cm) incisions. The surgeon performs the operation while viewing a high-resolution screen.

The closed method has many advantages over open surgery:

Much less postoperative pain

Smaller surgical scar (cosmetic advantage)

Less blood loss

Shorter hospital stay

Faster return to daily life and work

However, laparoscopic surgery is not suitable for every patient. It is generally preferred for cysts located on the anterior surface or edge of the liver—i.e., in an “accessible” location for surgical instruments—and for cysts that are not very complex (for example, not calcified).

How is the risk of hydatid cyst fluid leakage prevented in minimally invasive surgery?

In the early years of the laparoscopic method, the greatest fear was that the risk of cyst rupture and spread of contents into the abdomen would be higher while working in a closed environment. However, very special techniques and systems were developed to eliminate this risk.

Today, a special technique called a “closed system” is used. In this technique, a special instrument (trocar) is placed into the cyst. While one channel of this instrument perforates the cyst, the other channel simultaneously begins to aspirate the cyst contents with high vacuum. Thus, the pressure inside the cyst is suddenly reduced, and at the moment the cyst is punctured, leakage of its contents into the abdomen is prevented with “zero leakage.”

After the cyst is safely drained, the laparoscopic camera (since it provides magnification) is inserted into the cyst. The surgeon can see the inside of the cyst in high resolution, remove the parasitic membranes, and most importantly, detect and repair small bile leaks much more clearly than could be missed in open surgery.

Is the most feared postoperative problem ‘bile leakage’?

The most common and most troublesome complication of hydatid cyst surgeries (especially conservative surgery) is persistent postoperative “bile leakage,” i.e., Persistent Biliary Fistula. This is when bile coming from the cyst cavity or the cut surface of the liver continues to drain through the placed drain (tube) for days.

The reason for this is that, during the growth process of the cyst, it damages small bile canaliculi within the liver, or these canaliculi cannot be identified during surgery.

If bile leakage develops, is another hydatid cyst surgery required?

No—no longer. Thanks to modern medical technologies today, the vast majority of postoperative bile leaks can be treated with an endoscopic method called “ERCP,” without the need to take the patient back to surgery.

ERCP (Endoscopic Retrograde Cholangiopancreatography) is a procedure performed by a gastroenterology specialist under light sleep (sedation). A special endoscope is introduced through the patient’s mouth, passes through the stomach, and reaches the small nipple-like opening called the “papilla,” where bile drains into the duodenum.

During ERCP, the following procedures are performed:

The bile ducts are cleared

Obstructions are opened

A stent is placed

This stent is a small plastic tube that directs bile along the correct path (to the intestine). It is placed not into the leak site but into the main bile duct. Thus, the direction of bile flow changes, pressure decreases, and the load on the leak site is relieved. In this way, the leak site closes and heals on its own within a few weeks. After healing is complete (usually 6–8 weeks later), this stent is removed with the same simple method.

The availability of ERCP has made hydatid cyst surgery much safer and has provided the ability to resolve one of the complications surgeons fear most without surgery.

Why is follow-up important after hydatid cyst surgery?

Undergoing hydatid cyst surgery does not mean that you are completely free of this disease. This is an “insidious” disease that tends to recur. Therefore, postoperative follow-up is at least as important as the surgery itself.

The postoperative period is also part of the treatment. What needs to be done in this process includes:

Medical therapy (Albendazole)

6-month follow-up

Annual follow-ups (at least 5 years)

Regardless of the type of surgery and the status of the cyst, all patients receive postoperative (adjuvant) albendazole therapy. This treatment generally continues for several more months after surgery. Its purpose is to eliminate microscopic “seeds” (parasites) that may have been missed or spilled during surgery.

Patients are evaluated in the first 6 months after surgery with a follow-up ultrasound and blood tests. Even if everything goes well, these check-ups should be performed once a year for at least 5 years, and sometimes longer, to catch a possible recurrence at a very early stage or detect a secondary cyst that may develop in the abdomen.

Frequently Asked Questions

How Long Does Liver Hydatid Cyst Surgery Take?

Liver hydatid cyst surgery generally takes between 1.5 and 3 hours, although it varies depending on the size, number, and location of the cyst.
