It is performed both for diagnosis and treatment, and the removed tissue is sent for pathological examination.

In skin tumor surgery, the entire tumor is removed with safe surgical margins. When cancer is suspected, a wider excision is preferred. The procedure may be performed under local or general anesthesia and is usually completed in a short time.

The postoperative healing process varies depending on the size and location of the lesion. For small lesions, stitches may be removed within a few days, while healing can take longer after wide excisions. Regular dressing changes and proper wound care are important.

Cosmetic outcomes are also considered after skin tumor excision. With aesthetic suturing techniques, scarring can be minimized. The postoperative pathology report plays a critical role in determining the further treatment plan.

What You Need to Know

Information

Definition

Skin tumor excision is the surgical removal of benign or malignant tumors that form on the skin.

Purpose of Surgery

The aims are complete removal of tumor tissue, establishing a diagnosis (via biopsy), preventing spread, and improving cosmetic appearance.

Techniques Used

Local excision (with a surgical incision), Mohs surgery (especially for facial areas), electrocautery, and cryotherapy may be used.

Type of Anesthesia

Usually performed under local anesthesia; general anesthesia may be preferred for large lesions or in children.

Procedure Time

Ranges from 15 minutes to 1 hour, depending on the size and location of the lesion.

Hospital Stay

Usually done as an outpatient procedure with same-day discharge.

Healing Process

Wound healing typically takes 7–14 days; stitch removal is usually done within 5–14 days.

Postoperative Pain

Mild pain may occur and is usually controlled with over-the-counter pain relievers.

Risk of Complications

Risks may include bleeding, infection, widening or hardening of the scar, and tumor recurrence.

Success Rate

With early diagnosis and complete excision, success rates are high. Questions

What Is Surgical Excision and What Are Its Main Goals?

Surgical excision, in its simplest form, is the removal of a skin lesion using a scalpel together with a small amount of surrounding healthy tissue. This is almost always performed under local anesthesia, meaning only the area being treated is numbed with an injection. As a result, the patient does not feel pain during the procedure and can usually return to daily life shortly afterward.

This important procedure has several main goals:

Treatment

Definitive diagnosis

Preserving function and achieving the best cosmetic outcome

When a malignant tumor is present, the primary goal is to remove all cancerous tissue. This includes not only the visible tumor but also any microscopic cancer cells that may extend into the surrounding skin. For this reason, the lesion is excised with a “safety margin” of healthy tissue, which is vital for reducing the risk of recurrence.

At the same time, all removed tissue is sent to the pathology laboratory to establish a definitive diagnosis. Whether a lesion is benign or malignant can only be confirmed through microscopic examination, which is the gold standard and determines the next steps in treatment. Finally, especially in visible or functional areas such as the face, hands, or joints, modern surgery places strong emphasis on preserving function and minimizing scarring through careful planning and technique.

Is There a Difference Between Excision for Diagnosis and Excision for Treatment?

Yes. The intent of the procedure may vary—sometimes it is mainly diagnostic, sometimes mainly therapeutic, and often both. This is where an excisional biopsy comes in. In this method, the entire suspicious lesion is removed together with a narrow rim of healthy tissue. If the lesion is small and benign, or represents a very early cancer, this single procedure can both establish the diagnosis and complete the treatment.

In some cases the lesion may be large. Then, instead of removing it all at once, a small sample is taken first to identify what it is. This is called an incisional biopsy. After the pathology result is available, a definitive therapeutic excision can be planned.

If melanoma—a serious form of skin cancer—is suspected, the way the initial biopsy is performed can affect the entire treatment course. The recommended approach is to remove the whole lesion with a narrow margin. This allows the pathologist to measure tumor thickness accurately (Breslow depth), which is one of the most important factors for staging. If the first biopsy is excessively wide, lymphatic channels may be disrupted and this can complicate or even invalidate a sentinel (watchman) lymph node biopsy, which is often needed to assess spread. Therefore, the first biopsy is not only a diagnostic step but a strategic move that guides the correct treatment pathway.

Which Skin Lesions Are Treated With Surgical Excision?

Surgical excision is an effective solution for a broad range of benign and malignant skin conditions.

The most common malignant (cancerous) skin tumors include:

Basal Cell Carcinoma (BCC)

Squamous Cell Carcinoma (SCC)

Malignant Melanoma

Benign (non-cancerous) lesions are usually excised due to symptoms or to rule out cancer. class="ez-toc-section-end">

A successful procedure starts with good planning. Before the operation, several steps are taken to ensure both your safety and the best possible outcome.

First, a detailed medical assessment is performed. Your doctor may ask questions such as:

Do you have chronic conditions such as diabetes or heart disease?

Which medications do you take regularly?

Do you use blood thinners (aspirin, warfarin, etc.)?

Do you take supplements such as fish oil or vitamin E?

Do you have allergies to any medication, metal, or substance?

How have your previous wounds healed—do you tend to form raised scars?

Based on this information, decisions are made about issues such as how long before the procedure blood thinners should be paused.

Next comes surgical site planning. The direction of the incision is important to keep the scar as discreet as possible. Skin has natural tension lines. Incisions made parallel to these lines are exposed to less stress during healing and typically produce thinner, more aesthetic scars. Therefore, before anesthesia is administered, the lesion and planned margins are marked in a way that respects these natural lines.

Finally, the informed consent process takes place. The reason for the procedure, potential risks and benefits, alternative options, and the expected postoperative course are discussed in detail. This consultation—where your questions are answered and you feel confident about the plan—is a crucial part of treatment.

How Is Malignant Tumor Excision Performed?

When removing a malignant tumor, the key principle is to leave no cancer cells behind. Therefore, the technique is selected carefully based on tumor type and risk level.

The most commonly used approach is standard elliptical excision. In this technique, the tumor is removed in an elliptical shape with a predetermined safety margin, allowing the wound to close more easily and with better cosmetic results. For example, a 4 mm margin may be sufficient for a low-risk basal cell carcinoma, while a squamous cell carcinoma may require a 4–6 mm margin.

With malignant melanoma, the approach differs. Here, the main factor determining surgical margins is not the width of the tumor but its vertical depth (Breslow thickness) reported by pathology. Evidence-based guidelines define the recommended margins by thickness. For instance, a melanoma thinner than 1 mm is typically treated with a 1 cm margin, while a melanoma thicker than 2 mm often requires a 2 cm margin. This reduces recurrence risk without unnecessary tissue loss.

In melanomas above certain thickness thresholds, it may also be necessary to evaluate the first lymph nodes that cancer would likely spread to. This is called sentinel lymph node biopsy and is often performed in the same session as tumor removal. It is not a treatment itself, but a staging procedure that helps determine whether additional therapy is needed.

Do Surgical Methods Change for Benign Lesions?

Yes. Since the goal is not cancer clearance, simpler methods that preserve more tissue are often preferred.

Simple excision is commonly used for moles, cysts, and lipomas. Removing the entire lesion addresses the problem and allows pathology to confirm benignity. In cysts, removing the capsule intact is important to prevent recurrence. Lipomas can often be removed through a relatively small incision.

For some raised benign growths, shave excision (shave biopsy) may be used. With a special blade, the lesion is removed at the skin surface level, often without stitches and with a faster healing process.

Contact Us.

You can request an online appointment by filling out the form. In surgery, this follows a principle called the “reconstructive ladder,” meaning the simplest effective method is chosen first.

Wound-closure options may include:

Primary closure

Healing by secondary intention

Skin grafts

Local flaps

The most common method is primary closure, where the wound edges are brought together and sutured directly if there is enough laxity. Layered closure is often used for improved cosmetic results: deeper absorbable stitches reduce tension, and finer superficial sutures precisely approximate the skin edges and are removed later.

If the wound is too large to close directly, a skin graft may be used. A piece of skin is taken from another area (such as behind the ear or above the clavicle, where color/texture match is good) and transplanted to cover the defect.

For larger or more complex defects, local flaps are preferred. Nearby healthy tissue is shifted or rotated while preserving its blood supply to close the wound. This is especially valuable in aesthetic and mobile facial regions such as the nose, lips, and cheeks because it provides excellent color and texture match.

Home Care After Skin Tumor Excision

The success of surgery depends not only on the procedure itself but also on proper postoperative care at home. Following simple rules helps speed healing and improves the final result.

General care recommendations:

Keep the pressure dressing clean and dry for the first 24–48 hours.

For procedures on the head/neck, sleep with your head elevated for the first few days.

After the dressing is opened, clean the wound as instructed by your doctor.

Do not let the wound dry out and crust—keep it moist with the recommended antibiotic ointment or petroleum jelly.

Prefer acetaminophen (paracetamol)-based pain relievers for discomfort.

Avoid aspirin-like medications that may increase bleeding risk in the first few days, unless prescribed.

Avoid heavy exercise and strenuous activity for the period advised by your doctor (often 1–2 weeks).

Avoid swimming pools or the sea until stitches are removed.

What Can Be Done to Improve Scar Healing?

A scar can take up to a year to fully mature. During this time, a few measures can significantly improve its appearance:

Helpful methods include:

Sun protection

Silicone-based products

Massage

The most important step is sun protection. New scars are highly sensitive to UV exposure and can permanently darken. Applying a high-SPF sunscreen (SPF 30 or higher) regularly for at least one year is crucial. Once the wound has fully closed, and with your doctor’s approval, silicone gel or silicone sheets and gentle massage can help the scar become softer, flatter, and lighter over time.

Are There Risks Like in Any Surgical Procedure?

Although skin tumor excision is very safe, it carries potential risks like any surgery. These risks are rare but should be understood.

Possible risks include bleeding, hematoma (collection of blood under the skin), and infection. Infection is uncommon, but warning signs may include:

Increasing redness around the wound

Worsening pain

Swelling and warmth

Yellow-green, foul-smelling drainage

Fever

Less common risks include wound dehiscence (opening), nerve injury, and abnormal scar formation. Some temporary numbness around the incision is expected if small sensory nerves are cut and usually improves over time. Abnormal healing may occur in people with genetic predisposition, resulting in hypertrophic scars (raised but within wound borders) or keloids (raised scars that extend beyond the wound borders).

Why Is Postoperative Follow-up Important?

Postoperative follow-up is an essential part of treatment for two main reasons. First, it allows the doctor to confirm proper healing and remove stitches at the right time (usually 1–2 weeks after the procedure).

Second—and more importantly—if the removed lesion is malignant, follow-up serves an oncologic purpose. A person diagnosed with skin cancer has a higher lifetime risk of developing another skin cancer compared with the general population. Therefore, depending on the cancer type and stage, regular follow-up examinations (for example every 6 months in the first years, then annually) are vital. These visits help detect recurrence or a new lesion at the earliest possible stage.

Frequently Asked Questions

When is skin tumor excision necessary?

Skin tumor excision is performed when malignant or suspicious lesions need to be completely removed.
