In appropriately selected patients, it provides oncologic safety and offers more satisfying aesthetic outcomes.

Breast-conserving surgery is generally preferred in early-stage breast cancer. During the operation, the tumor is removed together with a margin of healthy tissue around it. This allows the cancer to be cleared while preserving the breast shape.

After breast-conserving surgery, radiotherapy is most often administered. This treatment reduces the risk of recurrence and increases the success of surgery. The combination of surgery and radiotherapy improves long-term survival rates.

In addition to its aesthetic advantages, breast-conserving surgery is also important psychologically. By preserving body integrity, patients often experience fewer negative effects in their social and emotional lives. The treatment process is supported with regular follow-up.

What You Need to Know

Information

Procedure class="ez-toc-section" id="What_Is_Breast-Conserving_Surgery">What Is Breast-Conserving Surgery?

Breast-conserving surgery is a surgical method for breast cancer treatment in which only the tumor and a margin of healthy tissue are removed rather than removing the entire breast. This approach aims to maintain oncologic safety while preserving the breast’s aesthetic integrity as much as possible. It is most commonly used in early-stage breast cancer and is typically supported by radiotherapy after surgery.

How Have Breast Cancer Surgeries Changed Over Time?

Years ago, when breast cancer was diagnosed, the standard treatment option was almost singular and very extensive: radical mastectomy. In this operation, the entire breast, the chest wall muscles, and all the underarm lymph nodes were removed. The belief at the time was that cancer spread like a stain and that removing more tissue would be safer. These operations undeniably saved lives, but the cost was high. Patients often had to live with lifelong arm swelling (lymphedema), restricted shoulder movement, and severe physical deformity.

As medical science progressed, we began to understand the nature of cancer better. It became clear that breast cancer does not spread only by local extension; even at diagnosis, it may already have reached distant areas microscopically through the bloodstream. This was a major turning point. It was understood that performing a very large local operation on the breast alone would not prevent microscopic spread and therefore did not always improve survival on its own.

This new understanding opened the door to a surgical revolution. Landmark studies in the 1970s and 1980s proved a critical fact in early-stage breast cancer: there was no difference in survival between patients treated with lumpectomy followed by radiotherapy and those treated with mastectomy. These findings clearly demonstrated that breast-conserving surgery is as safe as mastectomy in terms of cancer control. This was not just a shift in technique—it was a fundamental change in treatment philosophy. The goal became not only to survive, but to survive with a good quality of life.

What Are the Main Goals of Modern Breast Cancer Surgery?

Modern breast cancer surgery is built on three essential goals that must be achieved simultaneously. They work like the three legs of a tripod—if one is missing, success is compromised:

Completely Eliminating the Disease: This is the number one priority of any cancer surgery. The tumor is removed as a whole together with a “clean” (negative) rim of healthy tissue. This is the key condition for minimizing the risk of recurrence in the same area.

Minimizing Side Effects of Treatment: The aim is to keep treatment-related harm as low as possible. Avoiding problems such as arm swelling, chronic pain, and movement limitation—common after older operations—is essential. The best example is removing only a few “sentinel” nodes rather than all underarm lymph nodes.

Achieving a Good Cosmetic Outcome: This goal acknowledges the profound impact of breast appearance on body image, confidence, and mental health. The aim is to create a natural, symmetric, and aesthetically satisfying breast shape after surgery. This has led to advanced techniques such as oncoplastic surgery.

Who Is a Suitable Candidate for Breast-Conserving Surgery?

The decision for breast-conserving surgery requires careful evaluation and shared decision-making with the patient. Identifying the most appropriate treatment for each patient is the most important step of this process.

In general, patients with early-stage breast cancer (Stage I or II) whose tumors can be removed with clean margins without causing a significant breast deformity are ideal candidates. The ratio of tumor size to breast size plays a critical role. The patient’s desire to preserve the breast is also one of the most decisive factors.

Contact Us.

You can request an online appointment by filling out the form. It combines the strict oncologic principles of cancer surgery with the aesthetic principles of plastic surgery in the same operation. The goal is not only to remove the cancer, but also to leave behind a natural, symmetric, and aesthetically satisfying breast.

It is a mistake to see oncoplastic surgery as merely a “cosmetic touch.” In the right patient group, it can be more oncologically secure than standard surgery. Because breast reshaping is already planned, the surgeon can remove a wider margin of healthy tissue more comfortably. This significantly reduces the risk of positive margins and lowers the chance of needing a second operation.

Oncoplastic techniques vary based on breast size and the amount of tissue to be removed and are generally divided into two categories:

Volume displacement techniques: Used in medium to large breasts. After tumor removal, the defect is filled by rearranging and reshaping the remaining breast tissue, often combined with breast reduction or lift. For symmetry, the other breast may be adjusted in the same session.

Volume replacement techniques: Used in small breasts or when a large volume must be removed. Tissue flaps from nearby areas (often the back or chest wall) are used to fill the defect.

What Does “Clean Surgical Margins” Mean?

A surgical margin is the outer edge of the removed tissue specimen. After surgery, the specimen is sent to pathology and its edges are inked with a special dye. The pathologist examines whether cancer cells are present at the inked edge.

If there are no cancer cells at the inked edge, this is called a negative (clean) margin, indicating the tumor was most likely removed completely.

If cancer cells are present at the inked edge, this is called a positive margin, suggesting cancer may remain in the breast and often requiring re-excision surgery.

Today, accepted clean margin standards depend on tumor type. For invasive cancers, the key rule is “no tumor on ink.” For in situ cancers (DCIS), at least a 2 mm margin is typically targeted for added safety. These standardized criteria help avoid unnecessary repeat surgeries.

What Is Done for Underarm (Axillary) Lymph Nodes?

Just as breast surgery has become less extensive, axillary surgery has also changed dramatically over the last two decades. In the past, most underarm lymph nodes were removed to determine spread, often leading to lifelong lymphedema.

The standard approach today is Sentinel Lymph Node Biopsy. The lymphatic system works like a river network. If a breast cancer spreads, it usually reaches the first one or a few “sentinel” nodes—like guard checkpoints. During surgery, dyes or tracers are used to identify these nodes, and only they are removed.

If sentinel nodes are negative, the chance of spread to other nodes is extremely low, and no further axillary surgery is needed—protecting the patient from the risks of a larger operation.

Moreover, a major clinical trial (ACOSOG Z0011) showed that in breast-conserving surgery patients, even if 1–2 sentinel nodes contain small metastases, removing the remaining nodes is often unnecessary, because radiotherapy can control microscopic disease in the axilla. This was a breakthrough that spared many patients from avoidable surgery.

Is Radiotherapy Always Necessary After Breast-Conserving Surgery?

Yes—except for very rare exceptions, radiotherapy is an essential and standard component of breast-conserving treatment. Think of surgery and radiotherapy as a team: surgery removes the visible tumor, while radiotherapy destroys any microscopic cancer cells that imaging cannot detect.
