Surgical intervention is preferred in cases that reduce quality of life and do not respond to medication.

Laparoscopic hiatal hernia surgery is performed with minimally invasive methods. This technique provides less pain, a shorter hospital stay, and faster recovery. By narrowing the diaphragmatic opening, the stomach anatomy is returned to its normal position.

After hiatal hernia surgery, reflux symptoms decrease significantly. Complaints such as post-meal burning, chest pain, and chronic cough improve. This increases both digestive system function and overall quality of life.

After surgery, patients need to pay attention to their eating habits. Eating frequent small portions, avoiding carbonated beverages, and attending regular follow-up visits ensure the sustainability of surgical success.

What You Need to Know

Information

Description of the Surgery

Hiatal hernia surgery (hiatal hernia surgery) is a surgical procedure in which, when a part of the stomach slides through the diaphragmatic opening into the chest cavity, the stomach is returned to its anatomical place and the hernia sac is repaired. It is most commonly seen together with reflux complaints.

Types of Surgery

Nissen fundoplication (360° gastric wrap), Toupet fundoplication (270° posterior wrap), laparoscopic hiatoplasty repair, use of mesh (patch) when necessary.

Reasons for the Procedure

Large hiatal hernia (especially paraesophageal), severe reflux, difficulty swallowing, chest pain, anemia, gastric bleeding, hernias at risk of strangulation.

Duration of Surgery

Usually takes 1–2 hours; performed laparoscopically.

Type of Anesthesia

Performed under general anesthesia.

Length of Hospital Stay

On average 1–3 days; if no complications develop, the discharge time is short.

Recovery Process

Light activities are recommended within the first 1–2 weeks. Full recovery is usually achieved in 4–6 weeks.

Common Risks

Bleeding, infection, esophageal perforation, loosening of the gastric wrap, temporary difficulty swallowing.

Permanent Risks

Persistent dysphagia (difficulty swallowing), recurrence (re-formation of the hernia), difficulty passing gas, inability to burp, bloating.

Relationship With Reflux

Hiatal hernia often leads to gastroesophageal reflux disease. With surgery, reflux symptoms improve to a large extent.

Risk of Recurrence

The risk of the hernia recurring after surgery may be between 5% and 15%; it can be reduced with an appropriate surgical technique.

Alternatives to Surgery

If there are reflux complaints, medical treatment (PPI use), lifestyle changes. However, surgery is mandatory in large hernias.

Pre-Operative Preparation

Endoscopy, barium swallow study, pH monitoring test, esophageal manometry, pulmonary and anesthesia evaluation.

Post-Operative Follow-Up

Dietary regimen, monitoring swallowing functions, follow-up of gas and bloating complaints.

Diet

Liquid diet on the first day after surgery, pureed foods in the following days, then transition to soft foods. Returning to solid foods usually occurs within 2–3 weeks.

Return to Daily Life

After laparoscopic surgery, most patients can return to daily life within 1 week. id="What_Is_a_Hiatal_Hernia">What Is a Hiatal Hernia?

A hiatal hernia is a condition in which the upper part of the stomach slides through an opening in the diaphragm into the chest cavity. The most common type is a hiatal hernia. It usually presents with complaints such as reflux, heartburn, chest pain, and difficulty swallowing. Obesity, aging, and increased intra-abdominal pressure are among the risk factors. Mild cases can be controlled with medication and lifestyle changes; surgery may be required in severe cases.

In our body, there is a wide and strong muscle that separates the chest and abdominal cavities, stretching like a dome; this muscle is called the diaphragm. On this muscle, which plays a key role in breathing, there is a natural hole called the “hiatus,” through which the esophagus passes. Normally, this opening tightly surrounds the esophagus and prevents the stomach from slipping upward into the chest cavity. However, if this opening widens over time or the connective tissues supporting it weaken, the stomach herniates upward through this opening like a balloon. This condition, known in medicine as “hiatal hernia,” is commonly called a hiatal hernia.

So why does this structure weaken? One of the most important factors is age. As the years go by, our muscles and connective tissues lose their former elasticity and strength. This natural process also affects this sensitive area in the diaphragm. In addition, conditions that continuously increase intra-abdominal pressure can trigger or accelerate hernia formation. The main factors that trigger this condition are:

Excess weight or obesity

Pregnancy

Chronic and severe cough

Constipation that causes persistent straining

Heavy sports or jobs that require regular heavy lifting

Fluid accumulation in the abdomen (ascites)

We classify hiatal hernias into different types in terms of both the problems they cause and the treatment approaches. This classification guides us in determining the decision for surgery and the technique. The most common type we encounter is Type I, the “sliding type” hernia, which accounts for 95% of all cases. In this hernia, the junction where the esophagus meets the stomach slides above the diaphragm. The biggest consequence of this is that it disrupts the natural valve mechanism that prevents the acidic contents of the stomach from flowing back into the esophagus. For this reason, classic reflux complaints such as heartburn and bitter fluid coming up into the mouth are usually based on this type of hernia.

Other hernia types, which are rarer but can cause more serious mechanical problems, are grouped as “paraesophageal hernias.”

Type II Hernia: The stomach-esophagus junction is in its normal place, but the upper dome (fundus) of the stomach has slipped into the chest cavity alongside the esophagus.

Type III Hernia: Both the stomach-esophagus junction and the dome of the stomach have shifted together toward the chest cavity. This is the most common type within this group.

Type IV Hernia: This is the most advanced and complex type. Along with the stomach, other abdominal organs such as the colon and spleen have passed into the chest cavity through the enlarged opening in the diaphragm.

Because this second group of hernias has the potential to lead to urgent and life-threatening conditions such as the stomach becoming trapped, its blood supply being compromised, or twisting around itself, they should be monitored more closely.

In Which Cases Is Hiatal Hernia Surgery Performed?

It should be noted right away that not every detected hiatal hernia must be operated on. The decision for surgery is made entirely on an individual basis depending on the type and size of the hernia, the severity of the patient’s complaints, and whether these complaints can be controlled with medication.

In the most common Type I hernias, the decision for surgery is usually based on uncontrolled reflux complaints. The main situations in which surgery is considered are:

Severe heartburn and bitter fluid coming up into the mouth that persist despite strong acid-suppressing medications

Non-healing sores (esophagitis) or narrowing (stricture) in the esophagus due to reflux

Chronic cough, hoarseness, or asthma-like respiratory problems resulting from reflux contents aspirating into the windpipe

The patient being young and not wanting to use medication lifelong

Patients who cannot continue treatment due to side effects of medications

In paraesophageal (Type II, III, and IV) hernias, the situation is somewhat different. In these hernias, the decision for surgery is made by considering the mechanical problems and potential risks created by the hernia rather than reflux. The symptoms that bring surgery to the agenda for these hernias are:

Pain, pressure, or a feeling of tightness in the chest after meals

A feeling of food getting stuck when swallowing solid foods or difficulty swallowing (dysphagia)

Feeling full quickly and bloating even after very little food

Iron deficiency anemia due to small bleeding caused by the herniated stomach being compressed by the diaphragm

Shortness of breath (especially in large hernias due to pressure on the lungs)

So what should be done if a large paraesophageal hernia is detected with no symptoms? This is one of the most current topics of debate in this surgery. In the past, it was recommended that all of these hernias be operated on in order to prevent future emergencies such as strangulation or gastric volvulus. However, studies have shown that the risk of an asymptomatic hernia turning into an emergency is quite low. For this reason, the modern approach today prioritizes the option of “careful observation,” especially in elderly patients and those with additional health problems. The decision is made together with the patient by comparing the risks and benefits of surgery with the risks and benefits of waiting. However, conditions such as the herniated stomach twisting around itself (volvulus), which is accompanied by severe pain and vomiting, are emergencies and require immediate surgical intervention.

Contact Us.

You can request an online appointment by filling out the form. Patients can usually be discharged on the 1st or 2nd day after surgery. To ensure the long-term success of the operation, it is very important to carefully follow the rules in this early period.

The most critical part of the recovery process is nutrition. To avoid damaging the sutures in the surgical area and the newly created valve, the diet progresses in stages. The general stages of this diet are:

Stage 1 (First 1–2 weeks after surgery): Only liquid foods are consumed. Water, compote, strain-free soups, milk, ayran, and protein supplements form the basis of this period.

Stage 2 (Weeks 2–4): Transition to soft foods with a pureed consistency. Easily swallowed and digestible foods such as mashed potatoes, yogurt, vegetable purees, and scrambled eggs are preferred.

Stage 3 (After week 4): A slow and careful transition to normal solid foods is made. In this period, chewing food very well and eating slowly are essential.

There are some foods and drinks that should definitely be avoided in the first month of recovery:

All carbonated and sparkling beverages

Bread and pastries (due to their sticky and swelling structure)

Rice and pasta

Tough and fibrous meats

Raw vegetables and unpeeled fruits

In terms of activity, patients are encouraged to start walking immediately after surgery. Walking regulates blood circulation and speeds up healing. The most important restriction is related to heavy lifting. For the repair in the diaphragm to heal, it is necessary to avoid lifting objects heavier than 10 kilograms, straining, and movements that strain the abdominal muscles for at least 4–6 weeks. While desk workers can usually return to work within 1–2 weeks, for jobs requiring physical strength this period may take 4–6 weeks.

What Those Who Have Undergone Hiatal Hernia Surgery Should Pay Attention to

Hiatal hernia surgery offers you a new and healthy start. To make this start permanent and to ensure that the success of the surgery lasts a lifetime, it is vital to adopt certain lifestyle habits. These rules are designed to protect the repaired area and help the body adapt to the new anatomy. The most important points to pay attention to in the long term are:

Reduce your portion sizes and increase the number of meals. Eat 5–6 small meals a day instead of 3 large meals.

Give each bite its due; chew foods very well and slowly.

Drink plenty of water between meals, not during meals.

After eating, sit upright or take a walk for at least 1–2 hours; never lie down immediately.

Finish your last meal at least 3 hours before going to bed.

Maintain your ideal weight. Excess weight is the greatest enemy of the hernia by increasing intra-abdominal pressure.

Raise the head of your bed by 15–20 cm. This is very effective in preventing nighttime reflux.

Prefer comfortable clothing instead of tight clothes and belts that constrict your abdomen.

If you use it, quit smoking. Chronic coughing continuously puts pressure on your repair.

Eliminate gas-producing foods and drinks from your life as much as possible.

These simple but effective habits not only help you get through the post-operative period smoothly, but also guarantee that the comfort and health you have gained will stay with you for years.

Frequently Asked Questions

 Who needs hiatal hernia surgery?

Hiatal hernia surgery is preferred in patients who have reflux complaints that do not respond to medication, in whom the hernia puts pressure on the stomach, or causes respiratory problems.
