This surgical intervention restores bowel continuity and enables patients to return to normal bowel movements.

Whether the operation can be performed is evaluated based on the patient’s overall condition, intestinal healing, and the reason for the initial surgery. The procedure is generally planned and safe, but individual factors can affect success.

After the stoma is closed, temporary bowel irregularities may occur. Diarrhea, gas, and abdominal cramps are common complaints. Regular nutrition and close medical follow-up are important for a healthy recovery.

Potential postoperative complications include anastomotic leakage and infection. With early diagnosis and appropriate treatment, these issues can be managed. When performed by an experienced surgeon, the procedure offers durable, long-term solutions.

Things You Should Know

Information

Definition of the Surgery

Stoma closure surgery is a surgical procedure performed to end a previously created colostomy or ileostomy and restore bowel continuity.

Purpose of the Surgery

To close a temporarily created stoma after recovery is complete and allow stool to pass through the normal route again.

Eligibility / Conditions

The underlying disease (e.g., cancer, infection, inflammatory bowel disease) should be treated, and the bowel and anastomosis line must have healed.

Surgical Method

It can be performed with open or laparoscopic techniques. The bowel ends at the stoma site are reconnected (anastomosis), returned into the abdomen, and the abdominal wall is closed.

Type of Anesthesia

Performed under general anesthesia.

Operation Time

On average 1–2 hours, depending on the technique and the patient’s anatomy.

Length of Hospital Stay

Usually 3–7 days. id="What_Is_Stoma_Closure_Surgery">What Is Stoma Closure Surgery?

Stoma closure surgery can be compared to reopening a main road that was temporarily closed due to roadworks once all repairs are completed. Sometimes the body also needs a similar “detour.” For example, after a severe colon infection, perforation, or a high-risk surgery, stool flow may need to be diverted temporarily so that the affected bowel segment can heal safely. This diversion is achieved by bringing the bowel to the abdominal wall—creating a stoma.

Once the stoma has completed its job—meaning the bowel segment it was protecting has fully healed—it becomes unnecessary. Stoma closure surgery ends this temporary route and restores digestive “traffic” to its natural pathway. During the procedure, the surgeon carefully frees the bowel opening from surrounding tissues, restores bowel continuity, and returns the digestive system to normal function. This plays a key role in helping the patient return to normal life both physically and psychologically.

The extent of the operation depends on the type of stoma created initially. Closure of “loop” stomas, where a bowel loop is brought out, is usually simpler. Closure of “end” stomas, where a bowel end is completely externalized, typically requires a more complex operation.

When Is Stoma Closure Surgery Performed?

The decision to close a stoma should not be rushed and must be made jointly by the patient and the surgeon. Not every patient with a stoma is immediately suitable. For a safe and comfortable outcome, certain conditions should be met—much like a vehicle undergoing a full check before returning to the road after a long journey.

Key criteria suggesting suitability include:

Full overall recovery after the initial operation

A good nutritional status

Blood values returning to normal ranges

Complete healing of the bowel segment beyond the stoma

No leakage or problem at the anastomosis line

No active disease (inflammation/infection) in the lower bowel segments

No local recurrence in cancer patients

Adequate strength of anal sphincter muscles for continence

Timing is as critical as these criteria. In many cases, the optimal window for stoma closure is about 3 to 12 months after the initial surgery. There is a reason for this “golden window.” Performing closure too early (before 3 months) may be technically harder because tissues can still be swollen and not fully healed, increasing complication risk such as infection. On the other hand, delaying closure beyond a year may lead to increased intra-abdominal adhesions, which can prolong surgery and raise the risk of injury to surrounding organs. In cancer patients, additional treatments such as chemotherapy usually need to be completed first, because chemotherapy can temporarily weaken immunity and wound healing capacity.

Contact Us.

You can request an online appointment by filling out the form. Recovery begins in the hospital and continues at home as an adaptation period.

During the first few days in the hospital, the body starts to overcome the effects of surgery. Pain control is a priority. IV fluids are given to maintain hydration. One of the most important steps is early mobilization—getting up and walking as soon as possible. This helps bowel function return faster, reduces clot risk, and supports lung health. Nutrition is resumed gradually:

Clear liquids (water, tea, stewed fruit juice)

Soft, low-fiber foods (purees, yogurt)

Gradual transition to a normal diet

Discharge is planned when the patient can eat comfortably, bowel function returns (passing gas or stool), and pain is controlled with oral medication. This is typically 3–5 days for loop stoma closures and up to about a week for more complex end stoma closures.

After discharge, the main adaptation process begins. Patients should know it takes time for bowel function to reach a “new normal.” In the first weeks, diarrhea, frequent bowel movements, and urgency are common and usually part of normal adjustment. Interestingly, some patients may later experience a tendency toward constipation. In patients treated for rectal cancer, a condition known as Low Anterior Resection Syndrome (LARS) may occur due to reduced storage capacity of the rectum.

Frequent bowel movements

Urgency

Gas or stool leakage

A feeling of incomplete emptying

Fragmented bowel movements

These symptoms often improve over time and with dietary management, but realistic expectations and patience can make the process psychologically easier.

What Should Patients Pay Attention to After Stoma Closure Surgery?

Your active participation in recovery is as important as the surgery itself. Simple but effective habits—from diet to daily activities—can make recovery more comfortable and reduce problems.

Diet is the most important part of this adaptation. Give your intestines time to adjust without overloading them. Some foods can help, while others may cause discomfort early on.

Foods that may help firm the stool in the first weeks include:

Banana

Rice porridge

Mashed potatoes

Applesauce

Yogurt

Foods and drinks to reintroduce gradually or limit early on include:

Raw fruits and vegetables

Legumes (chickpeas, beans)

Cabbage, broccoli, cauliflower

Spicy foods

Fried and high-fat foods

Carbonated/acidic beverages

Alcohol

Other helpful lifestyle and activity recommendations:

Daily walking

Avoid heavy lifting for 6–8 weeks

Avoid straining

Drink at least 2 liters of water per day

Clean the anal area gently

Use protective barrier creams if needed

Finally, although recovery is usually smooth, some “red flag” symptoms require urgent medical attention. Contact your doctor or go to the nearest emergency department if you experience:

Fever above 38°C

Severe abdominal pain

Persistent nausea and vomiting

Inability to pass gas or stool

Excessive redness, swelling, or discharge at the wound site

Uncontrollable diarrhea

Frequently Asked Questions

When is stoma closure surgery performed?

Stoma closure surgery is performed when bowel continuity can be safely restored and the patient’s overall condition is suitable.
