During surgery, the diseased tissue is removed, thyroid function is preserved as much as possible, and the risk of complications is minimized. It is one of the core procedures in endocrine surgery.

Total thyroidectomy is the surgical removal of the entire thyroid gland. It is usually preferred in cancer cases and helps reduce the risk of recurrence. After surgery, thyroid hormone is replaced with medication, and lifelong treatment may be necessary.

Partial thyroidectomy is performed when only a portion of the thyroid gland is removed. This approach is typically preferred for benign nodules or disease confined to a single lobe, helping preserve some thyroid function.

After thyroid surgery, protecting the nerves that control the vocal cords is extremely important. Calcium balance must also be monitored. Regular follow-up after surgery is essential to check hormone levels and manage possible complications early.

Things You Should Know

Information

Definition of the Surgery

Thyroid surgery (thyroidectomy) is the surgical removal of part or all of the thyroid gland.

Indications

It may be performed for thyroid cancer, large nodules, hyperthyroidism (overproduction of hormone), multinodular goiter, or suspicious biopsy findings.

Types of Surgery

Total thyroidectomy (entire gland removed), subtotal thyroidectomy (most of the gland removed), lobectomy (one lobe removed).

Type of Anesthesia

Performed under general anesthesia.

Duration of Surgery

Usually between 1 and 3 hours.

Hospital Stay

Most patients stay in the hospital for 1–2 days after surgery.

Recovery

Full recovery may take 1–2 weeks. id="What_Is_the_Thyroid">What Is the Thyroid?

The thyroid is a butterfly-shaped gland located at the front of the neck. As part of the endocrine system, it produces hormones (T3 and T4) that regulate metabolism. These hormones affect many vital functions, from energy production to heart rate. When the thyroid does not function properly, health problems such as hypothyroidism or hyperthyroidism may occur.

What Is Thyroid Surgery?

Simply put, thyroid surgery is the operation to remove diseased thyroid tissue from the body. When performed by experienced and specialized surgeons, it is generally a safe procedure with highly satisfying outcomes. There is no “one-size-fits-all” surgery; treatment is individualized for each patient. The extent of surgery is determined by the type and spread of the condition. There are two main approaches: total thyroidectomy, where the entire gland is removed, and lobectomy (also called hemithyroidectomy), where only one lobe is removed. The goal is to completely remove the diseased tissue while preserving healthy tissue and protecting nearby structures as much as possible. In this way, we aim to restore health while maintaining the highest possible quality of life.

When Is Thyroid Surgery Needed?

The decision for thyroid surgery is made together with the patient after detailed examinations and evaluation. This is a step-by-step process, and each step guides the next. It usually starts with a neck ultrasound, which provides valuable information about the structure of thyroid nodules. If a nodule looks suspicious, the next step is fine-needle aspiration biopsy (FNAB). With FNAB, cells are taken from the nodule and examined by pathology to determine whether there is a cancer risk. Based on all these findings, the need for surgery is decided.

Main conditions that may require surgery include:

Diagnosis of thyroid cancer

Strong suspicion of cancer

Enlargement (goiter) causing pressure on the trachea or esophagus

Toxic goiter (hyperthyroidism) that does not respond to medication

Cosmetic concerns due to visible neck swelling

Suspicious nodules with inconclusive biopsy results

Thyroid Cancer or Strong Suspicion of Cancer

The clearest and most important indication for surgery is a confirmed thyroid cancer diagnosis by biopsy. Sometimes the biopsy does not definitively say “cancer,” but indicates a very high probability. These situations are also considered definite indications for surgery. The aim is to completely remove cancerous tissue and prevent spread.

Benign Conditions

A significant number of thyroid surgeries are performed for benign diseases. In such cases, the goal is to solve mechanical or hormonal problems caused by the condition. When the thyroid grows excessively (goiter), it can begin to compress surrounding vital structures and cause symptoms that reduce quality of life, such as:

Difficulty swallowing

Shortness of breath

A sensation of choking or a lump in the throat

Hoarseness

Snoring or noisy breathing

When these symptoms significantly affect daily life, removing the enlarged thyroid to relieve pressure is often the most effective solution. In other situations, the thyroid may produce too much hormone, leading to toxic goiter. If medication or other options fail, surgery can provide a permanent solution. Also, even if the biopsy is benign, surgery may be considered when a nodule is larger than 4 cm, because very large nodules carry a slightly higher risk of missing a small cancer focus on needle biopsy.

Nodules with Uncertain Diagnosis

Sometimes FNAB cannot give a clear answer and the result is reported as “indeterminate” or “suspicious.” For these “gray zone” nodules, a diagnostic surgery is often recommended. In this operation, the thyroid lobe containing the nodule is removed and examined thoroughly by a pathologist. This provides a definitive diagnosis. class="ez-toc-section-end">

Like any surgical procedure, thyroid surgery carries certain risks. However, it is important to note that when performed by an experienced surgical team, the rates of these complications are quite low. Before surgery, patients are informed in detail about possible risks and preventive measures.

The most common potential risks include:

Bleeding (Hematoma)

Hoarseness (vocal cord nerve injury)

Low calcium (Hypocalcemia)

Infection

Bleeding (Hematoma): The main early concern after surgery is bleeding and blood collection at the surgical site. Although rare, it may require urgent intervention because it can compress the windpipe. Therefore, patients are monitored closely during the first 6–12 hours. Sudden neck swelling, bruising, and shortness of breath can be warning signs.

Hoarseness: Very delicate nerves that control the vocal cords run directly behind the thyroid gland. Protecting these nerves is a top priority. If the nerve is stretched or becomes swollen during surgery, temporary hoarseness may occur and typically improves within weeks. In experienced centers, the risk of permanent hoarseness is below 1%. Intraoperative nerve monitoring is often used to help identify and protect the nerve, further reducing risk.

Low Calcium (Hypocalcemia): Small glands called the parathyroids (usually four) regulate calcium balance and are located very close to or attached to the thyroid. In total thyroidectomy, their blood supply can be affected or they may be unintentionally injured. This may lower blood calcium levels. Early symptoms can include:

Numbness in fingers and toes

Tingling around the lips

Muscle twitching or cramps

This complication is most often temporary and managed with calcium and vitamin D supplementation after surgery. Permanent low calcium is more common after total thyroidectomy and may require ongoing medication.

How Is Thyroid Surgery Performed?

Thyroid surgery is performed under general anesthesia, meaning the patient is completely asleep. The surgical technique and extent are carefully planned according to the disease type, thyroid size, and overall health status.

The key decision is how much of the thyroid to remove. In total thyroidectomy, the entire gland is removed. This is usually preferred for thyroid cancers, large goiters involving both sides, or toxic goiter that cannot be controlled with medication. In lobectomy, only the diseased lobe is removed. This is often used for unilateral benign nodules, indeterminate nodules, and—based on modern approaches—for selected low-risk thyroid cancers meeting certain criteria (smaller than 4 cm, no spread). The main advantages of lobectomy are preserving the chance of natural hormone production and nearly eliminating permanent low-calcium risk.

Different access routes may be used, and the surgeon selects the best option for the patient:

Conventional Open Surgery: Considered the “gold standard” worldwide. A horizontal incision is made along a natural neck crease. It is safe for all thyroid conditions and provides the widest view and working area. The scar usually heals very well and becomes minimally noticeable over time.

Endoscopic (Minimally Invasive) Surgery: Performed through a smaller incision using a camera and specialized instruments. It is suitable for selected patients with a small thyroid and nodule. It offers a smaller scar cosmetically.

Scarless Transoral Thyroidectomy (TOETVA): A modern minimally invasive technique with no visible neck scar. It is performed through small incisions inside the lower lip and can be an alternative for eligible patients who prefer no neck scar.

One of the most important technological aids during thyroid surgery is intraoperative nerve monitoring. It can be compared to a car’s parking sensors: just as sensors warn you as you approach an obstacle, nerve monitoring provides audio-visual feedback when the surgeon is near the nerve or when it is stimulated. This helps locate and protect the nerve, especially in anatomically challenging cases, and plays a critical role in minimizing hoarseness risk.

Contact Us.

You can request an online appointment by filling out the form. Recovery is generally smooth and relatively fast.

Hospital Stay: Most patients stay 1–2 nights in the hospital, depending on the extent of surgery and overall condition.

Pain Control: Thyroid surgery is typically not very painful. Many patients describe mild throat discomfort when swallowing. This is usually well controlled with standard pain medication.

Eating and Drinking: A few hours after surgery, once the patient feels well, they can start with liquids and then return to normal foods.

Mobility: Patients are encouraged to get up and walk as soon as possible. This supports recovery, improves circulation, and lowers risks such as blood clots.

Postoperative follow-up focuses mainly on two key issues: bleeding and calcium levels. The neck and wound are checked regularly for bleeding. Calcium monitoring is especially important after total thyroidectomy. A modern approach is to measure immediate postoperative iPTH (parathyroid hormone), which provides an early clue about parathyroid function. If the level is low, calcium and vitamin D supplementation can be started proactively before symptoms occur, making recovery more comfortable and safer.

What Should Patients Pay Attention to After Thyroid Surgery?

Long-term management after surgery depends on the reason for the operation. Follow-up differs for benign conditions versus cancer.

Patients Operated on for Benign Conditions

If the entire thyroid is removed, thyroid hormone must be replaced with a daily tablet. Once the correct dose is set, patients can live normally without restrictions. TSH levels are checked periodically to ensure dosing is appropriate. If only one lobe is removed (lobectomy), the remaining tissue often produces enough hormone and medication may not be needed, but annual TSH checks are still recommended.

Patients Operated on for Thyroid Cancer

Successful treatment depends not only on surgery but also on lifelong, careful follow-up. The goal is early detection of recurrence and maintaining long-term health.

Hormone Therapy: Thyroid hormone medication serves two purposes: replacing missing hormone and suppressing TSH. TSH can act as a “growth signal” for microscopic remaining cancer cells. Medication dose is adjusted to keep TSH low, but the degree of suppression is individualized based on cancer risk—milder for low-risk patients, stronger for high-risk patients.

Regular Follow-Up

Blood Tests (Thyroglobulin and Anti-Thyroglobulin): Thyroglobulin is produced only by thyroid cells. After total thyroid removal, it should be near zero and becomes a highly sensitive tumor marker. Rising levels can be an early sign of recurrence.

Neck Ultrasound: The surgical area and lymph nodes are monitored for suspicious structural changes.

Check-ups are usually more frequent at first (e.g., every 6 months). If everything remains stable and results stay clear, follow-up intervals are extended (yearly or longer). With this dynamic follow-up approach, most patients can live a healthy, normal life after completing treatment.

Frequently Asked Questions

 Who needs thyroid surgery?

Thyroid surgery may be recommended for thyroid nodules, goiter, thyroid cancer, or hormonal imbalance.
