id="How_does_the_wound-healing_process_work">How does the wound-healing process work?

When a wound occurs, an impressive and highly organized process begins behind the scenes. Within seconds, your body activates an “emergency action plan” to repair the damage. To understand it more clearly, you can think of wound healing as a four-stage construction project. Each phase builds on the previous one, and all steps work together in harmony.

In any construction site, what’s the first priority when damage occurs? Securing the area. The body does the same. Immediately after injury, blood vessels constrict to reduce bleeding. Then platelets rapidly gather and stick together to form a temporary “barrier.” Soon after, the clotting system reinforces this barrier into a stronger “concrete wall.” This clot not only stops bleeding, but also becomes the first “scaffold” on which repair cells can work.

Once the site is secured, the next step is debris removal. This is where the body’s “cleaning crew,” white blood cells, takes over. First, neutrophils arrive as the rapid response team, removing bacteria, foreign particles, and dead tissue. A few days later, macrophages appear—like heavy-duty machinery. Macrophages perform deeper cleanup and release chemical signals that call in the “engineers” and “builders” for the next phase. This inflammatory stage explains the mild redness, swelling, and warmth seen around a wound and is essential for healing. However, if inflammation lasts too long, the site never fully clears and healing stalls—this is exactly what happens in chronic wounds.

After cleanup, the main construction begins. This phase starts a few days after injury and can continue for weeks. Several key tasks happen at the same time:

Building new supply lines: The repair process needs oxygen, energy, and materials. The body forms new blood vessels to increase blood flow in the wound area, ensuring resources reach the site.

Laying the foundation and walls: Fibroblasts—“construction workers” of the body—produce collagen, the body’s “cement and bricks.” New vessels and fresh collagen form a bright red, healthy tissue that fills the wound bed. This is called granulation tissue and is one of the clearest signs that healing is progressing.

Closing the roof: While the wound bed fills, skin cells at the edges (keratinocytes) multiply and migrate, covering the surface like a roof, restoring the protective skin layer.

Once the main structure is built, the final finishing phase begins. This is the longest stage and can last months or sometimes more than a year. During this remodeling phase, collagen fibers that were formed quickly and irregularly are gradually broken down and replaced by stronger, more organized fibers. This strengthens the wound area and improves flexibility. However, the repaired tissue never regains 100% of its original strength—usually reaching about 80%. In the end, the scar takes its final form.

Do all wounds heal the same way?

Not at all. Just like not every construction project is the same, not every wound is the same. Correctly classifying a wound is crucial for choosing the best healing strategy. The most basic distinction is based on healing pace and wound behavior.

Acute Wounds (Fast projects):

These follow the body’s normal healing timeline and progress smoothly. Surgical incisions, kitchen cuts, or scraped knees are examples. In acute wounds, the body knows what to do. The main role of care is to support the process, keep the wound clean, and prevent issues like infection that can delay healing.

Chronic Wounds (Projects that won’t finish):

These wounds fail to enter a clear healing phase after a certain period (often around 4 weeks). This is not just a delay—it usually signals an underlying problem. The healing mechanism is disrupted. The most common chronic wounds include:

Diabetic foot ulcers

Venous leg ulcers (varicose ulcers)

Pressure ulcers (bed sores)

Managing chronic wounds is very different from treating acute wounds. The goal is not only dressing changes, but identifying and correcting what is blocking healing—such as controlling blood sugar, improving circulation, or removing pressure on the wound.

How is a wound assessed and evaluated?

Effective wound treatment begins with careful assessment—almost like detective work. By examining both the wound and the patient, clinicians identify barriers to healing and determine what the wound needs. This evaluation is dynamic and repeated during each dressing change. Several key features form the “identity” of a wound:

Location: A wound over a bony area may be exposed to pressure; a wound over a joint may be stressed by movement.

Size: Length, width, and depth are measured to track progress. Shrinking size is a good sign.

Wound bed color: Color is one of the clearest indicators of wound health. Bright, healthy red (granulation tissue) suggests healing, while yellow sticky tissue (slough) or a black dry covering (eschar) indicates dead tissue that must be removed.

Drainage amount and type: Some drainage is normal. Clear, odorless, small amounts are expected. Increased drainage, yellow/green color, thicker consistency, or bad odor may indicate infection.

Wound edges: Healing edges are pink and active, gradually merging with the wound bed. Rolled-in, hardened, or purple edges may suggest stalled healing.

Surrounding skin: Redness, swelling, and warmth around the wound can signal spreading infection.

Pain: A sudden increase in pain often points to a problem, especially infection.

Contact Us.

You can request an online appointment by filling out the form. The gold standard of modern wound care is maintaining a moist wound-healing environment. Contrary to old beliefs, letting a wound dry out and form a thick scab is not ideal. A dry environment slows cell migration, while a moist environment allows repair cells to move freely—almost like “swimming”—which accelerates healing and reduces scarring.

The basic principle is simple: “If the wound is dry, add moisture; if it is too wet, absorb excess moisture.” There is no single dressing that fits every wound. Selection should match the wound’s current needs. Common dressing types and their roles include:

Hydrogels

Hydrocolloids

Foam dressings

Alginates and hydrofibers

Hydrogels have high water content and are used to rehydrate dry wounds. They help soften dead tissue and support easier natural removal. Hydrocolloids work well for clean wounds with low to moderate drainage; they gel when in contact with wound fluid and maintain moisture.

Foam dressings are ideal for moderate to heavy drainage, absorbing excess fluid while maintaining the right moisture balance. Alginates and hydrofibers are “super absorbers” used for very high drainage or deep, tunneling wounds. Their fiber structure turns into a gel on contact with fluid, filling spaces and trapping excess moisture.

Which advanced technologies are used for difficult, non-healing wounds?

Sometimes wounds resist healing even with the best standard care. In such cases, advanced therapies that actively stimulate and support healing can be used.

Negative Pressure Wound Therapy (NPWT / VAC): Think of this as a smart “vacuum cleaner” for the wound. A special foam is placed into the wound, sealed with an airtight dressing, and connected to a device that applies controlled suction. This draws wound edges closer, removes excess fluid and swelling, improves blood flow, and strongly stimulates new tissue growth.

Hyperbaric Oxygen Therapy (HBOT): The patient breathes 100% oxygen in a pressurized chamber. Under pressure, oxygen dissolved in the blood plasma increases dramatically, delivering “concentrated oxygen” to tissues with poor blood flow. This supports new blood vessel formation and strengthens infection control, especially in conditions like diabetic foot ulcers.

Bioengineered Skin Substitutes: For large or deep wounds, laboratory-developed “skin equivalents” may be used. These can be collagen-based scaffolds (a framework for the body’s cells to grow into) or scaffolds containing living skin cells.
